You'll need to remove the battery and locate the sticker. On the sticker you should locate a serial number that is called an IMEI or ESN (depending on the phone). Write down the number and call the company that the phone was made for. You should see their name and/or logo on the phone; examples include AT&T, Verizon, etc.
Then, they can provide you with the code and how to put it in.
