It sounds like your card is from a batch that was manufactured a little too slim. Electronically, it may be fine, but it's not fat enough to keep it pressed firmly against the contacts. The accepted fix sounds silly, but usually works, slip a folded up piece of paper in the slot with the card to wedge it in.
