Contact the service provider and they'll either supply a PUK code or will send a new SIM plus PUK code. They will ask a few basic security questions to confirm that you're the owner.
If you claimed on an insurance policy after losing the phone, then the phone is now the insurer's property and you must contact them first. Occasionally they'll let you keep it at no charge, but usually they'll want payment for the phone if you wish to retain it.
