Track down the twain driver for that scanner.. thats the box that opens when you scan... google for the company, download the driver.. re-install tadahh. Still no luck? manually remove the old one.. use search to finfd the twain folder in your windows system (prob twain32) then re-install. Still no luck? Scanner has died.

Driver mismatch - the driver of the scanner has forgotten the location of the TWAIN driver because another software has been using it lately. Follow this
