Check to see if there are any gouges in the needle plate. Sometimes when the thread is feeding through the plate and there are rough patches, it shreads the thread. If there are, you can get a small file and smooth the hole out, depending on how bad, sandpaper can also work ok. If it isn't your plate, check your hook, if there is a burr on the end of the hook, that can cause this problem as well. Same thing, smooth with a fine piece of sandpaper, not to much though, be very gentle when smoothing the hook as the length of the point is important.

I have a Yamata Family Sew machine. It keeps shreading the thread and wont' produce stitch. Needle is size 17. How do you know 4-6 tension, there is no marking for tension?
