Sounds like you need a new battery rechargeable batteries can get damaged especially if they are over charged or get too hot or are too old. Some older units are not designed to protect against over current however most are these days.
The adapter is set to provide a constant current to recharge if a new battery starts experiencing the same problems you will need a new charger as this will be causing too much current and reducing the dielectric inside hence no charge being held and hence no volts.
Despite all this advice most Nickel Cadmium will be good for over 1,000 recharges.
