After pressing OK or waiting the 30 seconds, press navigation key to bring up screen again. I'm guessing you have a Motorola phone, but maybe not. You will get back to the security lock screen but there will be a button on bottom right that states Forgot pattern (or passcode). If you forgot your screen unlock pattern or passcode, tap Forgot pattern (or passcode). You will be prompted to sign in using the Google account you programmed into the handset during initial start up. If successful, you will move to next screen to create a new security lock. You can cancel out of that step and return to homescreen.

Try this if it's for an iPhone- this may or may not work depending on model and iOS etc. you may want to try other hacktivate videos specific to your model.
Hope this helps the only other way requires a computer and full factory restore.
I've managed to come across this from someone who called microsoft with the same problem (it seems quite common)
Go into settingsConsole settingsThen device settings (The one with the blue screen) Then go: left trigger, right trigger, x, y, left bumper, right bumper, left with left stick, up with left stick, right with left stick, x
If you have trouble use the pad instead of stick, but I could only get the stick to work..
