I just purchased the Vizio SV421XVT TV and noticed that the audio varies greatly from channel to channel as well as from a program to commercial. This problem seems to be worse with the TruSound feature turned on. With the TruSound feature off the audio works just like our crappy old TV that does not have any sort of sound level regulation.I have contacted Vizio about this problem but no joy yet. My TV is currently connected to a DishNet 522 DVR as the secondary TV, this means that the connection to the TV is via a coax cable to the TV's CATV port. Vizio support asked me to try another type of connection such as Component inputs or HDMI but that won't be possible for another week or two. I also connected a Sony Blu Ray DVD player to the HDMI port and noticed some sound level inconsistencies there as well but at the time I attributed this to the movie that I was watching. I'm not sure if Vizio has a problem with their TruSound feature or if I just happen to have a lemon...

I am also having the same symptoms for my SV421XVT. TV works great except all HDMI ports are not receiving signal. Devices work through the AV Ports, and TV reception is fine. New Roku box is also not working through HDMI but works with AV. Wish I had a solution to this.
On a another forum for another TV it was suggested that boards need to be replaced. I hope I can get these boards for a reasonable cost.
U getting a non respones from ur remote or the manual side panel tv controls buttons wright.U already tries resetting it back to factory setting.But the problems still the same.Yes,the tv Tuner signal board is the causes and the problems.Tries websites like Shopjimmy.com,Ebay.com to buy a refurbish tv Tuner signal board for the replacement.
First - connect DVD player preferably to the same input (unless you are using RF connector) and make sure that same issue does happens while you are watching movies. If it does - main board in your TV have to be replaced. If it does not - it still cable signal problem.
Try pressing the MTS button on your remote while on the channel that does not have audio and select stereo. Most likely this will be set to SAP (secondary audio programming) and there is no SAP programming on that show. This is not something that could be avoided. It was during the channel scan, it locked in the SAP audio broadcasts for some reason.
