this happened to me yesterday after recieving whatever lexmark update my computer downloaded. I restarted everything, computer and turned on/off fax and unplugged for a bit.
It seems to be working OK now.
I know that's lame dell-style, advice, but the restart worked for me.

The Device Monitor is the software responsible for sending the application list down to the printer. If it crashes (you see a popup saying there was a problem with it and it has to shut down), then you'll be unable to scan until you get it running again. The easiest way to do that is simply reboot, as the Dev Mon should start up when you boot up your PC. If the Dev Mon crashes when you start up the PC or every time you try to scan for the first time after rebooting, then that would indicate a potential conflict with some other device driver or something else on your system. If that's the case and either of you are willing to provide more info and gather some log files, let me know and I can put you in touch with an engineer at Lexmark who will dig further into this problem.

If you originally installed from a CD, try uninstalling (should be a shortcut in the Start menu) and then
downloading and installing the latest driver from the web. Lexmark had this problem with
some products and the web driver may have a fix for it.

SOURCE: Lexmark 5400 series won't scan.
The first thing I would do is remove the program from your computer, then download the latest drivers from lexmark: visit
choose your version of OS ( OS 10 X, 10.3, etc) and download the file.
Before you install the software in your computer, I would disconnect the printer's USB cable, and wait for the installation program to prompt you to connect it.
I believe that will take care of your problem.
