No recalls for the West Bend 6-quart Slow Cooker, Model #64356 have been reported on either the West Bend official website or by the CPSC (Consumer Product Safety Commission).
Unfortunately, West Bend's parts page for your model says the cover is out of stock. There is a "contact us" link near the bottom of the page, though. Maybe they have some idea when the lid will be back, or can suggest a replacement.
Unfortunately, you can't, at least not from the manufacturer. Their parts page for your model lists only the crock. I don't know of any other parts sources for West Bend. There's one of these for sale right now (10:30 PM, EDT) on eBay which includes the lid, but that's it. Sad to say, it looks like you'll need a new cooker.

You don't say what size your slow cooker is but you can check for replacement parts at www.focuselectrics.com. Click on West Bend then, Replacement Parts tab and see if you can find one that would work. Your specific model is not listed.
Go to the manufacturers website here (http://www.focuselectrics.com/catalog.cfm?dest=itempg&itemid=2255&secid=54&linkon=category&linkid=69) and you can order replacement parts for this model. Anything from the lid to the stoneware crock to the feet.
