I was in same situation.
Found RepairsUniverse.com offered full instructions how to fit as well as suppling the screen at a cheap price. You can find the guide on their website. http://www.RepairsUniverse.com, under the fitting instructions section.
I choose to fit it myself, but they also will do it for you for $30. My screen arrived quickly and my phone is up and running again!

I've replaced my screen once yeah for soon to be second time. Anyways. You have to slide the back side of your phone off remove battery. take out the screws then pry the keyboard away from the phone (my kit came with a valuable pry tool). Second is removing a few more screws. If my memory serves me correctly you need to remove a small circuit board and then remove the slide screen from the board. Again you have to pry the screen away and the rest should be simple.

T-Mobile should be paying for the cracked LCD screens, as I understand they ended up doing for their Dash units. My LCD cracked within a week, then the LCD on the replacement unit they sent me cracked within a month. I am still arguing with T-Mobile on who should be paying for the first unit, let alone the second. This is a design engineering problem, nit a user problem.
