The jack has 3 contacts on it One is for the microphone, one is ground and the third one is for the voltage applied to the microphone circuits. Easiest way is to get a headphone with a microphone on it . That way you won't get a squeal out of your speakers or an echo when using Skype.
Since the Headphone jack is soldered onto the motherboard in most cases you could remove it by unsodering it from the MB. However, replacing it might cause another issue. You can always install a sound card that would allow you to have additional headphone jacks.

If you look at the tip of your plug on your headphone cord, it will have 1 black ring, or 2 black rings. 1 black ring is mono, 2 black rings is stereo.
If you are using stereo headphones, and you're still having this problem, your headphone plug is not going all the way into the jack on your notebook. Press the plug all the way into the jack and the problem should be fixed.

most likely headphone jack is broken. option one is to take it to the apple store and have them repair it. option 2 is to go on e-bay and buy a replacement jack and try to do it yourself. best option is to sell it on ebay as broken/needs repair. Someone who knows how to fix them will buy it from you and you can use the money you get to buy a good used working iPod. There are always tons of nanos for sale on ebay.
