SOURCE: SGH-G800 . What is PUK code?
The Personal Unblocking Key (PUK) is a code that is required to unblock your phone when you have entered the incorrect PIN ten times (or three times depending on the manufacturer of the phone)
it is an added layer of security for your phone in case somebody tries to fiddle with it and change anything on it.

SOURCE: can't remember phone lock code
Puk code is needed when you have entered the pin code wrong 3 times.
If you have a phone lock code on it and you can't remember to code try the defaults 0000 1234 12345 00000 etc. if you can't unlock it contact your dealer with proof or pruchase or contact a samsung service center, you can get those details from www.samsung.com you will most likely have to send the phone to be unlocked.
you can try entering these two codes without a sim card in the phone. write them where you would write 911 phone should restart. then insert the sim card and try turning the phone on.
*2767*3855# = Full Reset
*2767*2878# = Custom Reset
If these don't help then contact samsung for unlock service you will most likely have to send the phone to a service center.

here is some info on PUK codes:
try taking battery and SIM out waiting 15 seconds and powering cell back on.
In your case I believe your SIM is now FROZEN or locked. You mostlikely will need to purchase a new SIM card.
A PUK code (Pin Unlock Key) is required when you enter an incorrect PIN 3 times. After entering the wrong PIN 3 times, the card will be locked and you will need to get a PUK code from your service provider to unlock it.
If the wrong PUK code is entered ten times in a row, the SIM card becomes invalid and you will need to purchase a new SIM Card. The phone will display "PUK blocked call operator".
