If your's is the "s" model there is no keystone adjustment provided. Otherwise the keystone adjustment is found under MENU then 1, Size and Position and under MENU 2, Image Settings. Also, refer to this excerpt from the manual:
NOTE: This function is not available in "S" models.
Adjust the keystone control if images appears non-rectangular or keystoned in shape-that is, smaller at either the top or bottom edge. The effect of keystone adjustment is similar to
physically tilting the projector forward or backward in relation to the screen, and it affects all sources.
NOTES: 1) Keystone is enabled for a source only if Size is 1000 and Size x Vertical Stretch is 1,000,000.
2) Keystone adjustment is not available in "S"
models. 3) Avoid using Keystone with Blanking.

Keystone not working

Manual: http://www.mitsubishi-presentations.com/wp-content/files_mf/mitsubishi_manual_xd250ust.pdf

Installation>Wall Screen>Keystone

The automatic keystone adjustment may not be carried out correctly

because of the ambient temperature and the installation

conditions of the projector and the screen. In such cases, correct

the keystone manually.

• When the projector is projecting images where acceleration is

present, such as in a vehicle and aircraft, the automatic keystone

adjustment may not function correctly. In such a case, set AUTO

KEYSTONE in the INSTALLATION menu to OFF and correct the

keystone manually.

• You can correct the vertical keystones. However, their adjustment
ranges are limited in such correction.

When the keystone adjustment is carried out, the adjustment

value is indicated. Note that this value doesn't mean a projection

angle.

• The allowable range of the adjustment value in the keystone

adjustment varies depending on the installation conditions.

• When the keystone adjustment takes effect, the resolution

decreases. In addition, stripes may appear or straight lines may

bend in images with complicated patterns. They are not due to

product malfunctions.

• Noise may appear on the screen during the keystone adjustment

because of the type of the video signal being projected and the

setting values of the keystone adjustment. In such cases, set the

keystone adjustment values in the range where the image is displayed

without noise.

• When the keystone adjustment is carried out, the image may not
be displayed correctly because of the type of input signal.

In projector menu you will find a setting called 'keystone correction'. Use horizontal and vertical keystone correction to correct the projected image.

The whole projection is not square when projected onto the screen. How do we square it up?

if it looks like a trapezoid, you need to keystone it. you can either use the keystone buttons on the top panel or go into the menu under the image adjustments and select the keystone feature.

The image disappears on my tv screen. A horizontal line appears.

You need to check the horizontal IC for deffects, possibly and most likely there are dry joints on the legs of the Horizontal IC. Have all the legs on the IC soldered nicely, this shud sort out your problems as you say you hit the TV then works. Try this before replacing the IC

If there any way to get a properly aligned image when recording a document or some other rectangular subject that is not straight?

When the camera is at an angle to a business card, document, whiteboard, or other rectangular subject, one side of the subject in the recorded image may appear longer than the side opposite from it. This is called "keystoning," which is caused by the difference in distance from the camera lens to each side of the subject. The side of the subject that is closer to the camera looks longer and the side further from the camera looks shorter. Using the camera's Business Shot Mode automatically eliminates keystoning and gives a normal shape to rectangular subjects. Though the Business Shot Mode is able to eliminate the effects of moderate keystoning, certain conditions may make automatic keystone correction impossible. Also note that the maximum Business Shot Mode image size is 1600 x 1200 pixels. If you have the camera configured to record larger size images, recording with the Business Shot Mode will produce a 1600 x 1200 size image. When the camera is configured to record images smaller than 1600 x 1200 pixels, the Business Shot Mode will produce images in accordance with the currently specified image size.
