It depends on where you live and what's on offer.
I have found that Vodafone is best then t mobile then orange.
But things change all the time, they compete against each other all the time, so sometimes one is better than the other.
It's swings and roundabouts.

Is your phone asking for a PUK code. PUK = Personal Unlocking Key. Personal Unblocking Key, or PIN Unblocking Key.

This can only come from your cellphone carrier and usually the company that still owns the phone under the agreement. Sometimes it is because you have put another SIM card in it, and they own the phone and need you to use them as carrier to get the price of the phone back, or you are in default of payment, or on a Pay as You Go plan, it is over expired. I can also happen if you have entered the unlock code wrongly three times. This often happens when a phone is stolen or lost and has been found by or sold to someone else who then try and use the phone. Wikipedia A personal unblocking code (PUC), also known as a PIN unlock key (PUK), is used in 3GPP mobile phones to reset a personal identification number (PIN) that has been lost or forgotten.
Most mobile telephones offer the feature of PIN protection. After switching on the phone, if the PIN security function is active, the user is requested to enter a 4-8 digit PIN enabling the phone's non-emergency calling functions. If the wrong PIN is typed in more than three times, either the SIM card, the device, or both become locked. They can be returned to their original unlocked state by entering a PUC, provided by the service operator after verification. If the wrong PUC is entered ten times in a row, the device will become permanently blocked and unrecoverable, requiring a new SIM card. Cellular phone users are therefore advised by most providers to keep their PUC written down in a safe place separate from the device.
The PUC (PUK) is a SIM-specific code assigned and provided by the service provider.
Once you enter the PUK code, you must set a PIN. The PUK and PUK2 are used to unlock the PIN and PIN2 codes respectively if your SIM card is blocked

The PIN unblocking key (PUK; sometimes called personal unlocking key) is a master security code for unblocking and resetting the personal identification number (PIN) for your SIM card. For added security, the PUK is unique to your SIM card. If your PIN code is active and you enter it incorrectly three times, your SIM card will be blocked. You may be able to do it from the website or your cellular provider. This is from Orange - T-Mobile in the UK.
Your phone can become blocked in a number of ways - one of them is if you enter your SIM card PIN number incorrectly three times in a row. To unblock your SIM card, you will need to use something called a PUK (Personal Unblocking Key).

To unlock the phone u call the customer service of vodaphone and ask the CODE to unlock the phone.

Step 1: Getting the mysterious unlock code
Persuading your phone network carrier to give you the unlock code to your cell phone may be the most difficult part of the entire process.
The code is a number entered into U.S. phones to allow a SIM card from a third-party carrier to work with your cell phone. Internet message boards are full of complaints about carriers withholding codes along with offers of software or services for unlocking. But I also found a number of postings from people who said they got the code from their carrier simply by asking.
I called AT&T as a customer and explained my situation: I was going to the U.K. for one week, and would like to avoid high roaming fees by using a local SIM card. I explained that I had no wish to permanently change carriers, as I had a two-year contract and was happy with their service. Could I please have the unlock code for my phone?
I was met with a little reluctance. The first customer service representative told me this would require her to "submit a case" for getting the unlock code and that once approved it could take up to a week for the "unlock code team" to figure out the code for my particular phone. Her supervisor then tried to sell me an international plan that would reduce the usual $1.29 per-minute charge to "only" 99 cents per minute. I promptly declined. He then told me that it could take a week to get the code because it must be sent from the manufacturer.

Step 2: Unlocking your phone, activating the SIM card
I chose Vodafone as my test case and went to its store in Paddington Station upon arriving in London. Gabriel, the Vodafone employee who helped me, was kind enough to let me verify that the unlock code and the Vodafone SIM card worked before he charged me for the SIM card.

Before doing anything, I confirmed my phone's IMEI by typing in "*#06#" and "send". AT&T had sent the unlock code for the correct IMEI.
Next, I removed my old SIM card, inserted the new prepaid Vodafone SIM card and turned on my phone. I was immediately prompted to enter my eight-digit unlock code and instructed to press "OK". Doing this made the SIM and my phone available for use.
Vodafone's activation process was simple. You activate, get your phone number, set up voice mail, check your balance and add to your balance by punching in a numeric code for each function and pressing send. The company gives you a little book and cheat sheet to remember the codes.
The SIM card kit, which costs $10 (5 pounds) and includes $2 worth of talk time and free weekend calls and texts, remains active as long as you use it at least once every nine months. This means you can reuse the same SIM card and phone number the next time you travel where that network is available.
There are several price plans to choose from, depending on variables like mobile TV, free nights and weekends, video or picture messaging, video calling, mobile broadband, unlimited monthly usage and texting. I went with the standard "Anytime" plan that was 60 cents for the first three minutes of the day and 20 cents after that. You could save more money by buying 70 texts for $10 and just texting.
I bought a 5-pound Vodafone "TopUp" voucher with cash and entered the 12-digit code it came with on my phone to add more money to the SIM card. The vouchers also are available via ATMs for the banks HSBC, Lloyds, TSB, Natwest or Royal Bank of Scotland. You can alternatively register your credit card with Vodafone and add minutes by texting.
Step 3: The road test. And: What about switching back?
The phone worked without a hitch. It was nice to be able to give people in the U.K. a local phone number for their convenience and also store their incoming numbers on my own phone. Also, phone numbers provided when calling information can be texted to your phone. That may not seem like much, but it can be much easier than struggling to scribble down a four-digit exchange and six-digit number dispatched by a fast-talking operator.
Upon returning home, I was a little nervous: Nobody from either AT&T or Vodafone told me how to switch back to my original service. Upon landing in New York, I simply removed the Vodafone SIM card, put my AT&T SIM card back in and powered on the phone. After a few minutes, my phone reconnected with the local network and I was back to my old service.
Things to consider
Your phone must be compatible with one of the networks used in the country you're visiting. My Cingular 8525 (AT&T 8525) is compatible with GSM 850/900/1800/1900; EDGE; UMTS and HSDPA. The major mobile carriers in the U.K. use GSM 900, GSM 1800 and 3G networks, so there was no problem.
Finally, there's no guarantee that your carrier or phone manufacturer will be cooperative in handing over the unlock code for your phone. A little extra patience while on the phone with the customer service representative and giving your carrier plenty of notice before your trip will likely help.

I currently have a Vodafone SIM but am switching networks. When I put the new SIM in, it doesn't work. I have been told I need to put a PUK code somewhere, but Vodafone isn't telling me anything on where...

The Vodafone SIM card will only work on the Vodafone mobile network. If you want to use different mobile provider then you need to purchase a new SIM card from that mobile provider.
If you intend to use your old mobile phone that was connected to Vodafone, then if this mobile phone is locked to Vodafone, then the new SIM card from a different mobile provider will not work in the old mobile phone unless it has been unlocked to allow you to use any SIM card/mobile provider.

The answer is that there used to be a connection before O2 and Vodafone started blacklisting handsets! Orange and T-Mobile have been blacklisting handsets for a long time (It is only recently that O2 and Vodafone also started blacklisting handsets).

NB Orange & T-Mobile always lock their handsets!(e.g. an Orange handset will only accept an Orange SIM and will not accept an O2, Voda or T-Mobile SIM)

So if you reported your Orange or T-Mobile handset missing to your network it became barred/blacklisted! BUT it was only barred on your home network. Therefore unlocking the barred handset would enable it to work on every network except the one it was originally locked too! Therefore the phone still had some commercial value, as it would function on at least 3 out of the 4 networks.

It wasn't long before Orange and T-Mobile began to combine their individual blacklist databases. Therefore a phone barred on Orange was also barred on T-Mobile and vice versa. Even at this point the barred handset could be unlocked and used but only on 2 out of a possible 4 networks (O2 & Vodafone).

The government eventually stepped in and forced O2 and Vodafone to update their systems and introduced the CEIR. Now that all the networks share a central blacklist database, even if a barred handset is unlocked it still remains useless on ALL UK networks!

O2 SIM card won't work in a Virgin mobile

This is because you can't put an other network sim card in a different network phone.
Example: Vodafone SIM + T-Mobile Phone = Wont work.                Vodafone SIM + Vodafone Phone = Work.
