You could also call the Service Provider from other number. Your Sim would not be damaged if you have tried to enter wring PUK less than 3 times. If you might have punched that, in this case you need to have one more SIM. Yes, you could ask for a SIM for the same number. There would be extra charges that you might need to pay.
