Thanks for using Fixya.
PUK codes can only be acquired from your mobile service provider. If you are using a pre-paid sim card.. try to check the manuals and leaflets found with the pre-paid kit. You should be able to see the PUK code in there. If you are using a post paid subscription, you can easily get your PUK from the carrier by just calling them up and verify the ownership for the post paid line. I hope this helps.
