it may sound absurd but this could mainly be because of a fault with the hard disk... i had the same problem but realized the hard disk had become faulty probably due to overheating. what you will need to do is see if you can successfully format it and try runnning your disks again.. if they are still giving you problems you mah have to purchase a 3.5'' hard disk from any computer store(its quite cheap) then jus follow your manual as to how to replace it from the back of the ps3... and voila.. your ps3 will work as good as new
