Correct me if I am wrong but if you have completely blocked your PUK code then the phone is as good as trash.
Try Flashing it via the connection cable and an updater client such as wotanserver.com. This may help it may not. The only other way it could be fixable is by professional work, either your phone provider or a shop. Good Luck.
