Could be a couple of things. Clean out the channels of the coffee machine. Clean out the upper pick that pierces the K-cup. Sometimes this can start with a defective K-cup...yes, it happens.

I have a K10 mini cup keurig brewer. It worked fine until this morning. When I try to brew a cup, it comes out the top. It does with this with or without a k-cup in. I tried to clean out the needles...

First: You need to check a Upper Rubber Gasket (fits around the top puncture pin). If it missing you will have leakage because here's no seal to protect leakage. You can buy it online or call Keurig Customer Service (if you still have warranty)

Second: Upper Rubber Gasket not making a seal with the K-Cup or Reusable Filter and the water and/or coffee grounds leak out. To fix it simply move Upper Rubber Gasket 1/4" down. This should close the gap between K-Cup and upper needle.

Mini keurig stops brewing and blows tops off k-cups

It may be time to descale. Most models will give you a message when it's time to have the mineral deposits cleaned.
HERES HOW TO DO THAT...
Empty water resevoir. Disable auto off. Pour approx 48 oz of undiluted white vinegar into resevoir. PLace large mug on drip plate, run brew cycle, dump contents of mug in sink. DO NOT USE A K-CUP.
Repeat this process until the add water light is indicated.
Let brewer stand for at least 4 hours while still on.
Discard any residual vinegar in the resevoir and rinse thorughly.
AFTER approx 4 hours...fill resevoir with water.
RINSE:
PLace a large mug on the drip plate and water brew cycle. Pour the contents of the mug into the sink.
REPEAT this brew process until the add water light is indicated.
YOU MAY NEED TO REPEAT THIS "RINSE" PROCESS if you notice any residual taste.

Why does water leak back into my resevoir when i brew a cup of coffee and i only get half cups?

If you are experiencing inconsistent brews, there may be some coffee grounds lodged in the needles that pierce the K-Cup packs. This video will show you how to SAFELY clean this area of the brewer

Coffee grounds in the cup

since you said usually... I suspect the rubber seal that surrounds the top needle is not seating well...
you can verify this by making a beverage with a k-cup and, just before the water starts, add a little hand pressure to the K-cup arm pushing down a little bit... not hard...
If it consistently makes a good beverage, then we focus our attention to that piece of the brewer...
when the consistency of the water changes, it tells me to look for leaks or unusual percolation of water...

you can verify the K-cup holder is ok... but another way to simulate the hand pressure is to put a shim of some sorts under the lip of the k-cup... like the thickness of a couple sheets of paper under the rim... not the actual k-cup... but the k-cup holder... raising it up a smidge is the same as the slight hand pressure...

I would look to see if the rubber seal has come loose around the top needle....

My machine only brews 1/2 cup of coffee. I have de-scaled twice within the last week and I get a full cup of vinegar/water, but as soon as I put in a K-cup I only get 1/2 cup of coffee out. The first cup...

I had some coffee grounds stuck up in the cone shaped hole that the water comes from at the top. I had to use a very long toothpick but it broke up some of the grounds that happened to get caught up inside. Before doing this, I went through 24 oz of vinager ;) .. the toothpick worked!

The machine leaks water all over the counter when starting brew

My keurig did the same thing, the rubber grommet around the part that punctures the cup gets grounds on it and prevents a complete seal. Pop of the grommet and clean it and it will be good as new.
