You power supply board is bad. This was most likely caused by a power surge.
I would change the whole power supply to fix the issue but you could also try replace any bubbled capicators for a cheap fix.
The capicators will be less expensive but many times you will have the same issue once again if there is extensive damage to the power supply so the best thing to do is just replace the whole power supply board.
