I format a disk, use it..then i try to record some

i format a disk, use it..then i try to record some more at a later point in the day, and it asks me to reformat it again. also, sometimes we turn the camera on and we go to play recently recorded videos and they are just gone. and they were there earlier.

If you are using a DVD R/W disk then try using disks from a different manufacturer and alo try using a different medium such as DVD-R or DVD+R DL as the camera will not be able to erase any recordings.
