You have to open the iPod and push the piece out from the inside. There is a stepby step guide too that explainshow to repair the jack as opposed to replacing it : iPod touch repair. There are also third party iPod repair companies that do it for you.

broken earphones are usually not repairable due to its miniature construction. i am sorry to say it's more likely you need to buy a new one. if you are really a person that loves music so much and pays attention to sound details, i suggest you buying a good quality earphones like Sennheiser. Though this has a trade off when it comes to price.
