If it is asking for a PUK code then you don't have to worry about anything. Please make sure you do not Enter a wrong PUK too many times. Or else you SIM card will be useless
Get get a PUK code Please call your service provider's customer care, give them your number and they will provide you a PUK code without any charge.
Please note: there is no other way to obtain a PUK.
