Try to check the monitor with a different PC. If it does the same its a monitor issue and needs to be replaced. If you dont have another PC, then try to go to BIOS of your PC and see if its the smae over there. If the display is still distorted in BIOS, then its a monitor Issue. Else you need to reinstall the graphics card driver from device manager or need to update the driver or reinstall the O/s.
