Are you sure it is the scope and not your eyeball? I cannot think of a reason for a scope to go in and out of focus with intervention of some sort. what caliber are you shooting? Heavy recoil can shake things loose both in the scope and in your head.

How do you make adjustments, towards the shot or towards center of the target?

You move the cross hairs to the shot. Set up your gun on a bench and start by firing at a target out at about 15 yards. Once you are "on paper" set your cross hairs on the center and fire a 3 shot group Adjust the scope to the group. if the group is down 3 inch and left 4 inch. Adjust your scope accordingly. Then Fire another 3 shot group, until you are shooting center. Every time you adjust your scope tap the barrel of the scope with a screwdriver handle to keep the cross hairs from sticking. Once on at 15 yards move out to your max range and adjust to that.

Why do i see a blury target when i look thu the scope with myglasses and it is clear without them

Because your glasses (prescription or reading) are configured to correct your "far" sighted vision deficiency. Even though the scope is located at a very close distance to your eye, the point your scope is focusing to/at is many? yards in the distance. If you're like many of us, you can see satisfactorily at a distance, but your arms aren't long enough to allow you to read a magazine?

My elevation and windage adjustments aren't moving my scope. I shot 4 times and my group is 1 inch. Problem is I adjusted my scope between all 4 shots. I'm consistently shooting 7 inches high and 3 inches...

This can only be due to a "broken" scope. To be sure, you can put your gun in a vice (cushion it) and put the crosshairs on a fixed point. Without moving the gun, crank the adjustment knobs a good 10-15 clicks in one direction each, and see if the crosshairs move from where you had them pointed.
If they do not, the scope must be replaced. If they do move, perhaps you should try adjusting the scope quite a bit more between shots.
The best way to sight it in requires a helper. Fire one shot with the crosshairs on the bullseye. Then, without reloading, put the crosshairs back on the bullseye, and have a friend adjust the knobs until the crosshairs move to the ACTUAL point of impact of that shot. It is essential NOT to move the gun while doing this.
This method allows me to sight any gun in with only 2 shots (and a few for confirmation, of course). Best of luck!

Cross hairs are tilted to right while scope is straight

Without seeing the rifle, my guess would be that either the rings were not tight enough or the lens came loose from the shot. It is rare depending on the quality of the scope. If the lens itself moved, it will not be waterproof or fog proof because the seal on the lens is not tight. If its under warranty then it should be replaced, if not then be care full not to get it too wet. If you had a suction cup, you could try to reposition it straight again.
