The only way to get the PUK code is to call the service provider(not the phone maker) and give them the number that is on the sim card,They will give you the code needed to unlock it.So just have the sim card in hand to read off the number when they ask.

If you entered a pin code under the security option in phone settings, you will need to enter the code in order to unlock your phone. If you can't remember it, you can go to phone settings and under security, restore factory settings. You will need to select Restore and Clear. You will lose all your phone numbers, and personal settings but the phone will revert to the way it was when you bought it. Good Luck.
