Sounds like an hardware issue with the iPhone. Try restoring it and setting it up as a new iPhone and try a test call to see if the issue persists or not, if it does, then its a determined hardware issue and you can get it replaced. Depending if the iPhone is still within its 12 months hardware warranty period, you can get it replaced for free, if its outside of this time, then you would need to pay the out of warranty service fee. iPhone 4 is $179 if you take it into an actual Apple Retail Store or $198.95 if you set up repair over the phone with the Apple Tech Support team due to their mail in options.

You don't give us much to go on so here are some general solutions:

Turn the volume up during the call.
Get a hearing aid
Take it to your AT&T store so they can assist.

Cannot hear any callers of imcoming calls from my iPhone 3G


Removed SIM card, sync iPhone with iTunes. Reinstalled SIM card and resynced again. Incoming calls can now be heard.

IPhone - Can't hear incoming calls. Does not ring or Vibrate.


I had a similar problem....it was usually after I unplugged the headphones....the phone thought they were still attached, therefore, nothing came over the speaker......try plugging in the headphones, then unplugging...that worked for me.
