Your Nokia does not have a PUK code. But your sim card does. Check your sim pack, it should have your puk on it. Please, take note that you are not to randomly guess the puk code, but enter the precise numbers, other wise, if your wrongly enter the number 10 times, then the sim would be rendered unusable.
If you cant find your sim starter pack, get another sim card, probably a friend's, of the same network as yours, call your mobile service operators, and inform them of your predicament.
Alternatively, you could visit any of their offices closest to you.

As with all PUK codes, you can only get them from your SIM card provider.

If you have an unregistered pre-pay credit SIM, then unless the PUK was included with the original SIM documentation then you're stuck as the company will only provide them to registered users. If that's the case, your fix is to abandon your SIM and all credit, data, and the phone number associated with it and to install another SIM which means you also get a new phone number.

PUK stands for Personal Unblocking Key. This is a uniqueeight-digit code specific to your SIM card. This is required when the PINnumber to unlock your mobile phone is entered incorrectly 3 times. You are thengiven 10 attempts to enter your PUK code. If an incorrect PUK code is enteredafter the 10th attempt, you will block your SIM card permanently and willrequire a new SIM card from your mobile phone provider.
PUK codes can be obtained through the following ways - The PUK code is in your SIM card documentation Starter Pack.If you do not have this then -Please contact your phone service provider.
Your cell phone service provider can give you the PUK code once you give themyour account details and they have confirmed your identity.

Regarding your Sim from Optus I believe that you will need to call Optus to obtain the PUK code, unless you still have the card where the Sim card was removed from. it will have a PUK at the back which you will just need to rub off to get your PUK.
You can try 1234, but if this does not work you will really need to call Optus since they would know better how to get the PUK for your Sim if we try entering a PUK code several times that we are not sure of, we may disable or damage the Sim card more. Call Optus for further details regarding the PUK.

In order to get the puck code you must contact the network operator.
Call the customer care of your mobile operator and by providing your sim card number(printed on sim card ) and mobile number. you will be given the puk code

PUK refers to Personal Unblocking Key. This is a Code which is offered only by Service Provider. There is NO charge. Call up the service provider and ask for the same and they would be glad to offer that.

Note - If you enter 3 incorrect PUK codes ( For Most Service Provider's In US. For Most Service Provider's in UK it is 10 ), SIM would be permanently damaged.

unlock code is what you give while changing the company set code.
if you dont change it this may be 12345 or 00000
but if you have blocked it
you can get puk no from your service provider.
with the help of that puk you can change the unlock code of your phone.
but dont forget to write the puk no of your sim and unlock code of phone on secure place

You may try default phone and security code to remove restrictions.
ur defalt phones codes is 12345 or 00000 u can use it
Default 4 Digit user code: 1234 or 0000
Default 6 Digit Security code: 123456 or 000000

Type *#06# at the mobile screen to know IMEI no of your mobile.
send the IMEI no to me and i will reply you with unlocking code.
