It means that you have entered PINs wrongly few times, PUK code is purely Service Provider dependent, no need to worry, all you have to so is to call your Service Providers (Like AT&T, T-Mobile...).
And ask them for the code, they will first authenticate few things, and will give you the PUK code, in return,
