WOW! what a process! I don't know how you kept track of all that but , good job! Try this. It's a maybe, but better than nothing,
Put a non-accepted SIM Card and turn it on. Type *#9998*3323#. Press Exit. You will get a new menu. Select Malloc Fail.

The following is a known process for Samsung phones to unlock the restrictions.
If you want to unlock your phone put a sim from another company then type *#9998*3323# it will reset your phone. Push exit and then push 7, it will reset again. Put your other sim in and it will say sim lock, type in 00000000 then it should be unlocked. Type in *0141# then the green call batton and it's unlocked to all networks. This code may not work on the older phones and some of the newer phones. If it doesn't work you will have to reset your phone without a sim in it by typing *#2767*2878# or *#9998*3855# (not tested)
If this doesn't work you will need to post the IMEI number which is located under the battery for further help.

I had this same issue. The response I got from Samsung is that only select AT&T store or Samsung themselves can unfreeze (reflash) a phone. Do not pay $18.99 for the "Samsung Remotre Client" software www.unlocksamsungonline.com. It is a scam. It doesn't work and it cannot work. They will not refund your money either. Two things to think about: 1st - any positive messages about the software are likely posted from the company itself. 2nd - call Samsung directly and they will reflash your phone for free including shipping. Although it takes a few weeks to get it back. It's really your only choice if your local AT&T store cannot reflash your phone. If you have had a similar experience with this company, please contact me about a group effort to contact the the FTC with complaints.
