If you still have the sim card's holder the PUK numbers is written from there. If this is a post paid sim, you may call your service provider and they can unblock it for you.

If the sim is asking for PUK code or PIN no contact the service provider .i.e . sim card company customer care.
If you tried both and if the sim is blocked sorry to say this you need to contact your service provider and buy a new sim but still you can retain your old number itself

First of all don't put some unknown pin numbers because it gives u three turns after that it will completely block ur sim.If u allready do that,get to ur service provider and get a new sim by paying some money,if u donot Get ur sim number which is printed on the sim.Call ur service provider,give them ur sim number and get PUK1 and PUK2 and enter them as they are requested by phone

I advised you to call the telekom operator of which SIM card you were using.. The operator will assist you to make your blocked sim to be recovered.
The operator will need the information at the back of your SIM card which is the numbers written at the back..

if you sim is block with the puk code you will need to contact your mobile service provider to have that done, if the phone needs to be unlock them you will need to either take it to a mobile store that unlock mobiles or try online at unlockitnow.com or topsony.com

You need a PUK ( Pin Unblocking Key) code to be able to unblock the counter of you PIN ( Personal Identification Number ) . The code can be found enclose on your sim package .If its not in their,then you have to call your service provider operator and request for that code,or you can go directly in their wireless center for fast approval.
When you already receive the code, just enter it on your mobile that ask for a PUK code,after unlocking it,enter a new PIN code and it will ask to re-enter it again for confirmation.be sure you encode the exact numbers to avoid errors.
