If you removed the sim card while the phone was powered on, it's possible that the card was damaged or the data was corrupted. You can try putting the sim card in an adapter and checking it with your computer. You may have to format the card again, then reinstall it in your phone, then power on your phone again.
for Nokia Symbian OS can't read a SimCard,it will be go to Offline Mode by it self. Clean up the Sim Connector at the Phone,and make Sure your SimCard is not Damage. i recomended to you to try to using another Good Simcard for Testing your phone.

The Simcard might gone bad due to some reason, try this sim to other phone, if it not works then issue another sim to your service provider.

The PUK code is for the sim card and is nothing to do with the phone,
The phone will work with another simcard,
It is your simcard which is blocked due to you entering the pin code incorrectly 3 times,you need to contact the provider of your simcard and they will be able to give you the PUK code to unlock your simcard for free or you can just use a different simcard in your phone
