Problem for Microsoft Outlook 2003 for PC

Problem loading an outlook form.

I am trying to load an outlook form (a leave of absence outlook form published in the organizational forms library). After clicking on Tool >> Forms >> leave of absence form, the page began to hang and shaky and load of form failed as weel as the whole outlook hang. Need to terminate the process and restart outlook.
