puk code is provided by your service provider itself, call your service provider means the one who provides you the mobile phone connection.
the following details are essential for the same

1) which mobile phone number the sim was carrying which was last put in your mobile phone , which caused the puk code.

2) security details of the person on whose name the sim card is issued , like the name, address, birth date etc

3) mostly if u have been provided with a MIN number at the start when taking your mobile phone services, you could navigate through their IVRS to the spcific slot and put down your mobile number and the puk unlock code will be provided to you.

4) Call up the help line number of your respective service provider and they should help you out as the PUK code is service provider specific.nothing to do with the manufacturer of the mobile phone, but it is specific to the SIM card.

you will need to check with your provider to see if they can activate caller id for you.
for this to work both parties must have caller id active on there service

I think you need to contact your network provider, cos sometimes they instructed to allow you to send and recieve messages.
