When you go to sign in sign in with your google acct. Or when it asks for your password leave the password space blank and click on forgot password. This should take you straight to resetting you password.

I don't know that there is a way to move all those things, it maybe better just to phone up facebook and explain to them your situation and i'm sure they'd be happy to help, they couild even change your name if need be so that it reflects your divorced name/maiden name.
I have no idea what you mean by they're on different internet servers, do you mean you have a new isp at your new home?, if so that shouldn't matter one bit.
