Top thread? Mis-threaded, wrapped around a thread guide or the spool pin, top tension too tight. If the thread is fraying/shredding, possibly a damaged or incorrect needle, a burr or sharp edge catching the thread, or old/bargain bin thread.
Suggest removing the thread completely from the machine.
Install a brand new needle (needs to be compatible with the thread and fabric being stitched).

ALWAYS RAISE the presser foot and rethread from the beginning.
Verify the thread path is correct.
Set the top tension to the midway point and retest the machine.
Tweak the top tension so the top and bobbin threads meet in the middle of the fabric.
AVOID old or bargain bin thread. You may also try a different brand of thread. Some machines work better with one brand than another. If you can see little fuzzies projecting from the thread, get rid of it. All those fuzzies contribute to lint build-up in your machine and is not the best type of thread to use.

You may check to see if there is a burr on the surface of the hook. If there has been a needle break, it could have hit the hook and put a rough spot on it. You can use a very fine emory cloth of some kind to remove the burr

I had a similar problem and then realized that the thread on the top had a **** on the spool it was catching on.

This sounds like a backwards needle. Take out the needle and when you re-insert it back in, make sure that put the flat side to the back.

Unthread the machine and start from scratch. After rethreading make sure your spool of thread has free play at the reel (not caught on anything) then check the thread tension make sure it's not too tight. Also check the bobbin is in place correctly and the bobbin has been wound correctly not too tight. If it is still happening it may be an adjustment to be done by a tech? Also try different threat, faulty thread can do that as well.
