I have exactly the same problem. I was told by Tmobile to reinstall the software and that has not helped one bit.
I had the same problem, even after updating the software per T-Mobile. After the third time of having this problem I demanded another phone and so far this phone has worked out. I think the problem is in the motherboard and how it was designed. Call your carrier and tell them the solution they had you carry out is not working and you would like another phone.
