You can double check the suggestion of the vidoe card by connecting your computer to another monitor. If the lines appear on the second monitor then the diagnosis is correct and the video card in your computer is failing. If the lines do not appear in the second monitor, then your monitor's screen is failing and you'll need a new monitor.

I have 7 different hortizontal lines appearing

Try changing the VGA cable between the computer and monitor.
If that does not fix it, try a different monitor.
When you say "different operating systems" are they on the same physical computer/graphics card? If so, and the cable/monitor swap suggested above does not change any thing, then it is probably the Graphics card, or possibly something in the system (bad power supply?) that is injecting noise on the bus to the graphics card.

If none of the above. Bad Monitor.

When screen comes out of screensaver/sleep mode,

It's a bad LCD screen or bad video card. If its a laptop, try to hook an external monitor to the video port in the back and see if it has the same problem. If it is the same problem, then you know its the video card and not the monitor screen. If you have a desktop PC that has this problem, its most likely the video card. First try removing the monitor and trying it on another computer to see if the monitor is bad. If not, then remove and replace your video card and restart. If same problem, replace video card with a new one.

Heavy white lines accross the screen

Replace it, it is going bad. I could tell you what would cause this such as bad driver IC etc. but bottom line is its five years old, you got your moneys worth out of it so bite the bullet and get a new one.

Solid line completly across screen, monitor is brand new

Your comp's video card refresh rate (in Hertz, abbreviated Hz) may not be totally compatible with the monitor. Check the properties of your video card, and change the refresh rate (you may have to look around for it) to 60 Hz. This is a pretty standard refresh rate that works with just about every monitor. If it's already there, I'm afraid someone else will have to try and help!

Hope this helps, and please rate my answer, good OR bad.

Good luck!

Horizontal lines

Horizontal blur can be caused by both a bad monitor or a bad video card/Card Slot on the motherboard.

The easiest way to check this is to connect the monitor to a known good system. If the problem follows the monitor to the good system, the monitor is bad and will need replacement. If it works fine on another system the problem is the video card/slot

Distorted white lines running across screen

Disconnect the monitor from the computer and turn the monitor on
it will display a diagnostic colorbar are the lines still there.
if not they are likely a cable or video card.
if they are still there have a tech evaluate the monitor.

Horizontal line throught the screen.

Sounds like it might be getting too hot and/or going bad. Try it on another machine and see what it does. Also try another video card because the video card could be going bad.
