Question about Microsoft Xbox 360 Console
So the password can be reset with a button combination made on your controller, but that sequence can only be given by the people at Microsoft Technical Support. Write down the serial number of the Xbox (find that on a sticker on the back of the machine), and call 1-800-4MYXBOX.
They will likely ask you several questions about who plays it, gamertags, billing address, and so on, but they will then walk you through how to reset the password.

SOURCE: reset family setting password
Here's what you need to do:
Turn on your xbox, go to System Settings. From there go to Console Settings. A window should appear that asks you for your password. If you don't know your password, then make up something. It will tell you your password is incorrect, click Reset Password".
You'll need to type in: YrYa
If this fails, then you need to follow these steps to reset your factory settings.
Turn off your xbox. Take your hard drive out (the bulge on the side of your 360. It says HDD on it.) by pressing the gray button, and sliding the piece out. Turn on your xbox, go to System Settings - Console Settings - System Info.
Press these buttons IN ORDER:
LT RT X Y LB RB Y (using the left joystick) go right ONCE A
After all of this is completed, a window should appear asking you if you would like to reset your system settings, press yes!
After all of this is completed, you'll need to turn your xbox off, put the hard drive back in, and turn it back on.
I JUST had this problem, and this is what tech told me to do... and it worked.

From what i have gathered the only way to reset the master reset code is to contact microsoft and they can do it for you
The only codes I know are
Clear All Installed Game Updates and Console Cache
1. Go to the "system" blade
2. Select "memory"
3. Press Y on the HD symbol
4. Press X,X, Left Bumper, Right Bumper, X,X
5. A message will appear saying: "Do you want to perform maintenance on your Xbox 360 storage
6. Select Yes
Clear Any Failed system updates that cause the console to error.
1. With the console off, press and hold the sync up button (the small white one)
2. While holding the sync button press the power button to turn on the console
3. Continue to hold the sync button until the Console has booted up completely.
If none of the above suggestions work, contact MS customer support get get a support ticket started. If your
console is still under warranty DO NOT attempt to open it and fix it yourself. MS will fix it promptly and for free
and if you open it you loose the free support.
