It's not the phone per se, it's actually the SIM card. Your SIM card is asking for the PUK code because it has reached the maximum allowable amount of SIM code tries. If you bought a pre-paid kit the PUK code usually is provided in the package, better yet if you're on a post-paid line the PUK code will be provided by your carrier.

SD cards have a small slide switch along one edge. The position farthest from the contacts locks the card, protecting it from writes. The position nearest the contacts unlocks the card. If the switch is already in the proper position, slide it fully the other way and then back again.
PUK codes can only be acquired from your mobile service provider. If you are using a pre-paid sim card.. try to check the manuals and leaflets found with the pre-paid kit. You should be able to see the PUK code in there. If you are using a post paid subscription, you can easily get your PUK from the carrier by just calling them up and verify the ownership for the post paid line. I hope this helps.
