I'd gaurantee, although I don't know which as there are tens of thousands, a corrupt or an actual worm in one of the explorer extensions.
Also Verclsid.exe validates all shell extensions before they are started. Check out this MS article on programs which if installed will cause just what you are seeing: http://support.microsoft.com/kb/918165
