PUK number is related to your sim card and not to ur mobile . PUK code can be given only by your service provider which is specific for each and every mobile number. You will have to give some identification as proof .
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. If you cant contact through your phone get some other phone and contact them.U can also get it from the nearest service provider .
NOTE: You should ask your service provider,get the PUK,if you provide PUK code wrongly for 7 time, you sim may become useless, then you have to buy new SIM

hmmm that is one for the support folks at your email server company. Will require intervention on their end to correct due to security issues
sad to say but it's the new double opt in security most email services use these days

If there is a local branch I reccomend that you go in and speak to someone at the bank itself. If this continues to happen you can also look to close the account and move to a different bank. I would also call social security and inform them of the problem if you are unable to resolve it at the bank an seek reimbursement for back missing funds. Give them the new infomation from the new bank's account and they will let the old bank refund the money to them, however you will need to try and get the old account closed and likely you will be able to recover the funds at that time and without having SS do anything but change to a new bank and new account. This all applies only if you get direct deposit.
