Register the new card here. You may find that it says you already have an account based on your email address, in which case you can click for a password reset. Then you can add the new card as an additional card on the account. Then report the first card as lost.

How do I change my name and address on an existing panera bread card?

That seems more like an issue that you could find from the Panera Bread website or from a Panera Bread cashier. That is not an appropriate question for a laptop forum site.

Also, considering you would need to change your name and address on an existing card, there is probably no way to do that without creating a new card due to the fact that you are changing the two most crucial parts in the card, but I'm sure you will figure this out if you care enough to (pc problem forums won't be able to help)
