The sim card holder has a tiny slide mechanism. After inserting the sim card and pressing it against the body of the phone, place your finger over the card and slide it gently forward. The tiny plastic part that holds the card will snap it into place. Hard to explain, but it will work.

Either you need to put a code in the phone to accept any new sim card or the the phone sounds like it has been reported lost or stolen or the previous sim card had the wrong pin number inputted causing the phone to lock and then the right PUK number was not entered. The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.
