I have the same chair, and it gave me exactly the same diagnostic codes. Cozzia support told me that I needed a new backrest actuator, backrest pad, strength pad, and backrest harness. They offered to ship it to me for $345.00. I'm still trying to contact them to ask how to install these things, if there is any warranty, and why I need new pads.
Update:
I turns out that the only part I really needed was the backrest harness. I am rather upset with Cozzia customer service for sending me parts that I neither needed nor ordered. They did eventually refund me for the unnecessary parts, though.
Error Codes:
03 - backrest harness
08 - backrest harness
17 - backrest harness
25 - backrest actuator
28 - sensor within the 3D board on the backrest harness
The backrest harness is a long cable ($75) that is attached in various places throughout the backrest. I was able to repair my chair, but the process was NOT easy. Taking the back cover off the backrest was very difficult. Then, replacing the actual backrest harness cable was even more difficult. In the end, my chair is working again, and it cost me about $250 to fix - including paying a professional to complete the repair that I attempted to do myself.
