Seal installed wrong way round
groove worn in seal surface on axle /shaft
worn bush /failing bearing for shaft not keeping shaft in the center of the seal
Most commonly the Torque Converter where the seal rides has a grove worn it, check that first, also check for play in the shaft input shaft bearing..

it is also possible that you are not installing the transmission correctly
input shaft seals are easily damaged when putting the transmission into the torque converter
the correct method is to put the torque converter on the shaft first , the install the assembly onto the engine, flex plate while holding the converter in place
