How to unlock phone requesting PUK number
First of all, you need to know what PUK means, it simply means Personal Unblocking Key.

Phone requests for PUK code/number when a wrong code has been entered more than 3 times while trying to access your phone. The Sim card blocks itself and requires you to enter the PUK number in order for you to gain access to your Sim card once again.

Most of the times, the PUK number is found inside the pack that you got the Sim card out of. In the pack, you will see a card, the one you broke the Sim card from, you will see your mobile number, the Pin number to the phone, then you will see a 8 digit code on the card, that is your PUK number.

If you do not see it on the card you broke the Sim card from, then you will have to check thoroughly in the pack for the number.

If you do not find it there, do not panic, you can call the customer care of the Sim card provider, they will give you Your PUK number as long as you can answer some easy questions about your Sim.

Then you enter the PUK number on your phone, it will prompt you to enter a new PIN number, then you confirm the Pin number, after reentering the new Pin number, you will be able to assess you phone once again.
