PUK code can be given only by your service provider and you cant get these code anywhere in websites. Each and every mobile has a individual PUK code. Contact your SIM customer care department over phone. Its free of cost to get the pin or puk number. You can also use your neighbour phone if your mobile is not working.
The other option is you can visit the the nearest service outlet of your service provider they will help you for sure. Everything is done for free of cost.
