It's a Sears keyless entry system Model 139.53775
Are you sure it's hardwired? I have a 12yr old kenmore, and it uses a 9volt battery to power the key pad.

To program the keypad,
first unplug the garage door opener from the electrical outlet. Next,
remove the nameplate from the console that the keypad plugs into and
locate the program/operate switch. Flip the switch to program. Next,
enter your desired four-digit PIN number into the keypad. Slide the
switch back to operate and plug the garage door opener back in. The
keypad should be programmed.

If this unit can be operated with a wireless remote, then you should be able to purchase a universal wireless battery operated keypad. To determine if it is wireless look at the lift unit and see if there is a antenna (thin insulated wire) hanging from the unit. If so, go to Sears or any bigbox home store and purchase a universal wireless remote. You may also have corrosion on the wires inside the outside keypad. There may be a few screws hold the front and back of the keypad together. Before taking this off make sure power has been disconnected to the unit. Mine (battery operated wireless remote) had corroded the wires from the battery to the remote and I had to replace the wires.
